This session will be focused on supporting participants understanding of how to creating a quality and compliant Post-Secondary Transition Plan (Section 5) of the Individual Education Plan (IEP). The focus of the training will be geared towards the areas identified as needed support based on a self assessment at the beginning of the training and participants' comfort with information reviewed in the Universal Supports powerpoints. Please note that the purpose of this session is NOT to train exclusively on the new IEP form; however, the new forms will be used and referenced throughout the session.
All participants are asked to bring a recently completed IEP Section 5 for self-reflection. Records will be for your use only and contents will be kept confidential.
